Outreach/Home-Based Clinician:
-
Provides In-Home, Virtual and/or community based behavioral health support, psychoeducation, assistance, and clinical case management to caregivers/parents and families receiving services from the Department of Children and Family (DCF).

Conducts psychosocial assessments, and safety planning.
-
Develops treatment plans based on families’ needs.
-
Implements an eclectic, supportive, and multicultural approach that fosters support to assist the families in overcoming challenges.

Works collaboratively with DCF, Lead Agencies, and other care providers to ensure care coordination is being met and implemented.

Timely documentation of service in the client’s chart on the EHR system.
-
Master’s degree in social work, counseling, human services, or marriage and family therapy is required.

Parent Support Specialist:
-
Educates and supports parents/caregivers to engage in culturally appropriate and healthy parenting to strengthen parenting and family relations.

Provides supportive parenting coaching, role modeling, and emphasis on skill building training.

Services are provided in the home, community, virtual and office settings.
-
Connects the family with community resources as needed.
-
Works collaboratively with clinical team, including DCF, Lead Agencies, and other care providers to ensure care coordination is being met and implemented.

Timely documentation of services provided is required.

A bachelor's degree or five years of work-related experience is required.

Therapeutic Mentor:

Supports youth in developing confidence, essential life skills, and healthy social and emotional functioning.

Provides individualized coaching, role modeling, and skill‑building through structured activities in home and community settings.

Engages youth in recreational and social experiences that promote communication, problem solving, and positive decision‑making.

Services are delivered in the home, community, virtual, and office settings.

Collaborates closely with families, in‑home therapists, and behavioral health providers to ensure consistent support and aligned therapeutic goals.

A high school diploma with experience, or an associate or bachelor’s degree, is required.
